To start your follow up program:
Create an account and complete your profile to personalize the content of your Heart Plus application.
When the application is ready (content will be automatically unlocked within 24 hours), go to the « Objectives » section. According to your priorities, select one or more «Follow up Program».
This will activate the notifications, alerts and educative game modules in relation with your selected target.
It is recommended to choose few targets at a time, reach them and then activate others.
If you wish to enter data at your convenience, use the “Add value” section.
Monitor your progress on the validation charts; keep track of your health records and generate simple detailed reports to better communicate with your medical team: Directly on the application or on the secure website www.heartplusapp.com
